Personal Support & Care Planning at Seaton House
The Personal Support & Care Planning (PSCP) Program is offered in partnership with the City of Toronto and began as part of the city’s broader ‘George Street Revitalization (GSR)’ project. The PSCP program is located at Seaton House, Canada’s largest shelter for homeless men, offering an assisted living model of personal support work and coordinated care, working with older men and men with complex health issues.
The staffing team works in collaboration with city staff and partners at Seaton House to focus on the delivery of personal care services such as bathing, hygiene, transfers, toileting, and grooming care. With higher needs clients, staff also build a broader psychosocial client engagement and goal setting process around the full scope of activities of daily living.
Staff also work to build trust with clients, and refer those who are living with HIV/AIDS to additional supports and services – including that which are offered more broadly by Fife House – to create a pathway out of chronic homelessness toward stable housing and improved outcomes.
