Toronto Linkage to Care Program
The Toronto Linkage to Care Program provides direct service and care coordination to people diagnosed with HIV in settings where there is not a direct link to ongoing care. Toronto Linkage to Care works collaboratively with healthcare providers and community based organizations to develop pathways for referring individuals to affordable medical care and treatment regardless of immigration status, through short-term case management and connections to other supports including legal, immigration, housing, employment, mental health and substance use services. The Toronto Linkage to Care Program prioritizes applicants who self-identify as Black, Indigenous, or a person of colour (BIPOC). The program also explores supporting individuals who have challenges accessing traditional models of care, emphasizing community-based and interdisciplinary approaches to service delivery.
